ব্যবহার

সাইক্লোপেন্টোলেট প্রধানত মাইড্রিয়াসিস এবং সাইক্লোপ্লেজিয়া রোগ নির্ণয়ের উদ্দেশ্যে ব্যবহার করা হয়।

ফার্মাকোলজি

Cyclopentolate is an anticholinergic agent that induces relaxation of the sphincter of the iris and the ciliary muscles. When applied topically to the eyes, it causes a rapid, intense cycloplegic and mydriatic effect that is maximal in 15 to 60 minutes; recovery usually occurs within 24 hours. The cycloplegic and mydriatic effects are slower in onset and longer in duration in patients who have dark pigmented irises.

সেবনবিধি

ডায়গনিস্টিক উদ্দেশ্যে মাইড্রিয়াসিস এবং সাইক্লোপ্লেজিয়া তৈরি করতে, প্রাপ্তবয়স্ক এবং ১ বছরের বেশি বয়সী শিশুদের মধ্যে: চোখে ১ ড্রপ দেয়া হয়, তারপরে ৫ মিনিট পরে, প্রয়োজনে দ্বিতীয় ড্রপ। ডায়গনিস্টিক পদ্ধতি শুরুর ৪০ থেকে ৫০ মিনিট আগে ড্রপ প্রয়োগ করা উচিত। সিস্টেমিক শোষণ কমানোর জন্য, প্রয়োগের পরে ২ থেকে ৩ মিনিটের জন্য ল্যাক্রিমাল থলিতে আঙুলের চাপ প্রয়োগ করা উচিত। বিকল্পভাবে, ইনস্টলেশনের পরে চোখের পাতা ২ থেকে ৫ মিনিটের জন্য বন্ধ রাখতে হবে। প্রচণ্ড পিগমেণ্টেড আইরিসেস সহ রোগীদের বেশি ডোজ প্রয়োজন হতে পারে। ২৪ ঘন্টার মধ্যে মাইড্রিয়াসিস এবং সাইক্লোপ্লেজিয়া থেকে সম্পূর্ণ পরিত্রান পাওয়া উচিত।শিশুদের ক্ষেত্রে ব্যবহার: বাচ্চা এবং অল্পবয়সী শিশু এবং স্বর্ণকেশী চুল বা নীল চোখযুক্ত শিশুরা সাইক্লোপেন্টোলেটের প্রভাবের প্রতি বিশেষভাবে সংবেদনশীল হতে পারে, চিকিত্সার সময় পার্শ্ব প্রতিক্রিয়া হওয়ার সম্ভাবনা বাড়িয়ে দেয়। শিশুদের মধ্যে সাইক্লোপেন্টোলেটের ব্যবহার মানসিক প্রতিক্রিয়া এবং আচরণগত সমস্যার সাথে যুক্ত।

পার্শ্বপ্রতিক্রিয়া

Blinding acute angle-closure glaucoma and raised intraocular pressure may occur during cyclopentolate therapy. The mydriasis may be reduced by the intraocular application of pilocarpine, physostigmine or isoflurophate. Transient burning sensation of the eye is more likely with the 0.5% solution. Systemic effects, resulting from excessive absorption from mucosal surfaces or from ingestion of the drug, may include xerostomia, flushing, tachycardia and urinary retention. More severe systemic effects are tachypnea, scarlatiniform rash, delirium, psychosis, fever, stupor, coma, respiratory failure and death.

সতর্কতা

Cyclopentolate may cause increased intraocular pressure. Angle-closure glaucoma may be induced by cyclopentolate in patients with higher susceptibility to increased intraocular pressure including the elderly and individuals with shallow anterior chambers.Cyclopentolate may cause an increase in intraocular pressure which if sustained, can potentially lead to irreversible loss of vision. The drug should be discontinued and the physician consulted immediately if eye pain, blurring of vision, rapid pulse or dizziness occurs. Patients may require the use of dark glasses following the application of cyclopentolate, due to photophobia associated with mydriasis. Patients should be advised to contact their physician if blurred vision and photophobia continue for more than 48 hours after discontinuing cyclopentolate. Systemic absorption of topical cyclopentolate from the nasal mucosal surfaces may result in systemic adverse effects. This is particularly the case in children, who are most susceptible to the drug's adverse effects. If signs of systemic toxicity appear, such as dry mouth, tachycardia or dizziness, the dosage schedule should be reduced or the drug discontinued.
